29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about 02/27/2014 at the jobsite 1500 Olive, St Louis, MO, employees engaged in operations lowering metal cabinets 56 feet to the ground using a waste management recycle bag on a pulley system. Not all employees on ground level were wearing hard hats.
