29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:     4616 North Broadway, St. Louis: Employees were exposed to imminent danger where they were exposed to floor holes measuring approximately 14 inches by 13 inches and about 10 feet above a lower level which were not covered, or protected by guardrails systems, or employees were not required to use personal fall arrest systems.
