29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Johnston Landfill: On, or about, 02/25/2014 the employer did not instruct each employee in the recognition, avoidance and prevention of unsafe conditions in that the operator of the Frame Type Dump Trailer did not assess the wind speed and direction, surface conditions, and ensure that the payload was evenly distributed prior to dumping the trailer whereby the combination of adverse conditions resulted in a roll-over event.
