29 CFR 1910.1030(f)(1)(ii)(D): The employer did not ensure that all medical evaluations and procedures including the hepatitis B vaccine and vaccination series and post-exposure evaluation and follow-up, including prophylaxis, are provided according to the recommendations of the U.S. Public Health Service current at the time these evaluations and procedures take place, except as specified by paragraph (f). a) Alden Poplar Creek Rehabilitation and Health Care Center, Inc., Hoffman Estates, IL- Employees consenting to and being provided the hepatitis B vaccination series by the employer are not provided post vaccination serologic testing for anti-body to hepatitis B surface antigen (anti-HBs) one to two months after the final dose of the vaccine is administered, in accordance with the current recommendations of the U.S. Public Health Service guidelines. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1030(h)(5)(i): The employer did not establish and maintain a sharps injury log for the recording of percutaneous injuries from contaminated sharps: Alden Poplar Creek Rehabilitation and Health Care Center, Inc., Hoffman Estates, IL- The employer did not maintain a sharps injury log for percutaneous injuries from contaminated sharps including a needlestick injury to the employee's right palm that occurred on October 16, 2013; a needlestick injury to the employees right palm from an insulin syringe that occurred on October 5, 2013; and a needlestick injury to the employee's left second finger from an insulin syringe that occurred on July 11, 2013. No abatement documentation or certification is required for this item.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information: a) Alden-Poplar Creek Rehabilitation and Health Care Center, Inc., Hoffman Estates, IL- The employer has not provided to employees working with chemicals such as environmental services (housekeeping) staff using "Spectrum Select Short Cut Very Aggressive No Rinse Stripper" containing monoethanolamine and corrosive sodium hydroxide, "Ski Dew Bowl and Restroom Cleaner" containing phosphoric acid, and kitchen staff using "TOG oven & and grill cleaner" containing sodium hydroxide and butoxyethanol and "Zap disinfectant sanitizer" containing quaternary ammonium chloride the required training on the revised Hazard Communication Standard label elements ( e.g., pictograms and signal words) and the new format of the Safety Data Sheet (SDS) (i.e., section order and types of information contained within each section of the SDS), which was due December 1, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
