29 CFR 1910.216(b)(1)(iii): Safety tripwire cable or wire center cord was not located within 2 inches of a vertical plane tangent to the front and rear rolls. The tripwire cable or wire center cord did not operate readily whether the cable or cord was pushed or pulled:       a.       On or about March 19, 2014 and prior thereto at the 2 X 2 Breakdown Mill the trip safety trip cable at the feed side of the mill was approximately 6 inches from the  vertical plane tangent to the smooth roller , exposing employees to the hazard of not being able to reach the safety trip cable if they were caught by the rubber or in the bite of the mill.       b.       On or about March 19, 2014 and prior thereto, at the 2X2 Breakdown Mill the safety trip cable at the feed side of the mill did not operate when pulled up in that the overhead conveyor interfered with it being pulled up enough to activate the switch.  This condition exposed employees to the hazard of not being able to stop the mill if they were caught by the rubber or in the bite of the mill.       c.      On or about March 19, 2014 and prior thereto, at the 2X2 Breakdown Mill the safety trip cable at the back side of the mill was more than two inches from the vertical plane tangent to the corrugated roller, exposing employees to the hazard of not being able to reach the safety trip cable they were caught by the rubber or in the bite of the mill.

On or about May 8, 2014 and times prior thereto at the cracker mill, stop distance measurements were taken with rubber on the rolls.     b.  On or about May 8, 2014 and times prior thereto at the 2X2 breakdown mill, stop distance measurements were taken with rubber on the rolls.
