29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury, and the defective or damaged item was not removed from service: a) For the faciility at 3760 New Tampa Highway, Lakeland, FL 33801, as observed on or about 02/26/2014, portable cord and plug type drills and other power tools were available for use, and the outer insulation to their power cords was damaged and torn. Damaged power tools were available for employee use on an as-needed basis.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein: For the facility at 3760 New Tampa Hwy, Lakeland, FL, as observed on or about 02/26/2014, chemical containers at the facility containing hazardous chemicals were not labeled: a) Two unlabeled compressed gas tanks were present in the equipment storage closet. b) An unlabeled container of marking chalk was present on the main floor of the facility.
