29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about February 27, 2014, at Tubespec, LLC, the employer failed to ensure that employees wore protective equipment while driving a Toyota forklift. A forklift driver was operating a forklift without the use of a seatbelt. The forklift was equipped with a seatbelt.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: On or about February 27, 2014, in the pipe thread department, the employer does not ensure that compressed air used for cleaning purposes has been reduced to less than 30 psi. The air pressure regulator was set to 120 psi, the air pressure was measured at the end of the nozzle to be between 60 and 70 psi.
