The sharps injury log did not record the type and brand of the device involved in the injury, such as, but not limited to, "Instrument" (6-27-13), "IV Needle" (9-16-13, brand not recorded), "Instrument" (10-15-12), and "Sharp Instrument" (1-3-12).  Abatement certification and abatement documentation is required for this violation.
