29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    Shipping and Receiving Area, Cardboard Baler.  Employees were exposed to point of operation hazards during the operation of the cardboard baler because:     a)  The metal mesh safety gate/guard was not working properly.  The gate raised and lowered with the ram, and did not prevent employees from getting their hands or arms struck by the ram or the gate itself when the ram and gate descended.    b)  The metal mesh gate/guard had an opening in the lower left corner of the gate that was large enough for an employee's hand or arm to fit through.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
