29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph. Located along the rear interior area of the commercial structure adjacent to a stairway observed in use there is an energized Square D breaker panel board #NF442L6C without the dead front trim panel cover installed exposing live electrical parts therein operating at 110v or greater.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. Located along the rear interior area of the commercial structure there is a 480v SAES #780 Pro Floor Grinder and GE transformer #918368872 observed in use energized powered connected to electrical service panel by black flex cord to a Square D panel board # NF442L6C with the dead front trim panel not in place and transformer conductors to energized panel device without a strain relief coupler/clamp on the flex cord to prevent direct transmission pull to conductor joints at the panel.
