29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: On or about February 23, 2014, the employer did not select the correct personal protective equipment for a line break on a PFA tubing leaching system exposing the employee to skin contact with approximately 250 ml of 96% sulfuric acid.

29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed: On or about February 23, 2014, a PFA tubing leaching system being worked on was not checked for accumulation of stored energy before a first line break was conducted, exposing the employee to approximately 250 ml of 96% sulfuric acid.
