29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), were dispensed into containers without the nozzle and container being electrically interconnected.     On or about February 26, 2014, and at times prior thereto, in the flammable liquids storage areas Class I flammable liquids were dispensed into a smaller container without being effectively grounded and bonded to electrically interconnect the containers to prevent static discharge during dispensing operation. Employees were exposed to fire hazard.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:     On or about February 26, 2014, and at times prior thereto, at the micro finish area employees may be exposed to sulfuric acid, an eye wash capable of providing 15 minutes of flushing was not in working condition. Employees were exposed eye injury hazards.

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):    On or about February 26, 2014, and at times prior thereto, in the CEP and Sta-Kul areas employees were operating mechanical power presses where the "point of operations" were not effectively guarded. Employees were exposed to an amputation caused by the presses while in operation.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    On or about February 26, 2014, and at times prior thereto, in the punch press area employees were exposed to sprocket wheel and chain systems approximately 5-ft. from the floor without a guard provided to prevent employees contacting the sprocket wheels and chains while in operation. Employees were exposed to an amputation and caught in hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    On or about February 26, 2014, and at times prior thereto, at the establishment electrical equipment was observed being used not in accordance with the listing instructions, including but not limited to the following instances:     1) In the small disc area a four-plex electrical receptacle was observed being used where it was laid on the floor and not mounted to a wall or structure.     2) In the punch press area a relocatable power tap (RPT) was observed being used to supply power to large current consumption equipment.     Employees were exposed to electric shock and electrical fire hazards.
