29 CFR 1926.1101(k)(9)(vi): Training for employees performing Class IV operations did not include available information concerning the locations of thermal system insulation and surfacing asbestos containing material and/or presumed asbestos containing material, asbestos-containing flooring materials, or flooring material where the absence of asbestos has not yet been certified; and instruction in recognition of damage, deterioration, and delamination of asbestos containing building materials; and did not take at least 2 hours: a) Maintenance & repair work involving doorbell transformer(s) and main water line valve(s) is performed by employees within the crawl spaces under buildings, where old pipe insulation is present. The age and nature of this thermal system insulation on the pipe(s) meets the criteria of presumed asbestos-containing material (PACM); for which maintenance staff was not instructed as to recognition & reporting evidence of damage / deterioration. Condition(s) were noted on or about 2/28/2014. **NOTE** Employee(s) who perform repair & maintenance duties where ACM / PACM is likely to be disturbed must be trained in accordance with requirements of Class III Asbestos work; and shall be consistent with EPA training for local education agency maintenance and custodial staff set forth at 40 CFR 763.92(a)(2), and shall take at least 16 hours.
