29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a) At Building 9, a tube and coupler scaffold was not inspected by a competent person prior to the work shift.  Employees were performing stucco work at heights greater than 30 feet.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) At Building 9, employees were exposed to falls greater than 30' while performing stucco work on the east side of the chimney from a temporary, elevated work platform without fall protection.  b) At Building 9, on 02/27/2014, employees were exposed to falls greater than 30' while performing stucco work on the west side of the chimney from a temporary, elevated work platform without fall protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) At the site, employees performing stucco work 30' above the ground were not protected by any type of a fall protection system.
