29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  On or about February 27, 2014, the employer did not conduct frequent inspections to provide a workplace where any employee, worker, laborer or mechanic employed in the performance of the contract were free from recognized hazards such as but not limited to:  a) workers performing remediation work on exterior panels of a building were exposed to an unprotected fall hazard approximately 18 feet to the ground below;  b) workers accessing the roof area by use of a ladder that was not at least three feet above the upper landing surface and exposed to a fall hazard approximately 18 feet to the ground below; and  c) workers operating a circular saw were not wearing safety glasses to protect from eye injuries by flying objects.
