29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Cumberland Farms building, 502 Washington Street, Auburn, MA; On February 27, 2014 employees were exposed to unprotected falls of approximately 14 feet while working on a roof with a pitch of roughly 8 in 12.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Cumberland Farms, 502 Washington Street, Auburn, MA - Roof; Employees were exposed to falls of approximately 14 feet while accessing the roof from a ladder that did not extend 3 feet above the point of access.
