29 CFR 1926.102(a)(1):    Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:                                                                                                            (a) On or about February 27, 2014 at worksite 9304 NE 93rd Kansas City Missouri.- employees were not wearing eye protection to prevent an injury to the eyes when using a pneumatic nail gun.

29 CFR 1926.501(b)(13):   "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  (a) On or about Febuary 27, 2014 at worksite 9304 NE 93rd Kansas City Missouri-. Employees were exposed to serious injuries, in that the unprotected sides and edges of the elevated work area was not provided with guardrails or any other means of fall protection. The employees were exposed to an approx. 13 to 18 ft fall hazard to the surface below.

29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about Febuary 27, 2014 at worksite 9304 NE 93rd Kansas City Missouri-  Multiple emloyees were  exposed to a fall hazard by using a ladder which did not extend 3 feet above the landing surface of the roof to which they gained access.
