29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about February 27, 2014, employees were working from an aerial boom lift platform at heights of approximately 20 feet above the ground were not wearing fall protection. The employees were exposed to fall hazards.

29 CFR 1926.602(c)(1)(viii): A lift truck, equipped with vertical only, or vertical and horizontal controls elevatable with the lifting carriage or forks for lifting personnel, did not have the following precautions to include (A) Use of a safety platform firmly secured to the lifting carriage and/or forks: On or about February 27, 2014, employees were working from a shop made steel platform on an all-terrain forklift truck that was not secured to the lifting carriage. Employees were exposed to fall hazards of approximately 20 feet to the ground below.
