29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.     a) 3rd floor:     Employees used an electric powered circular saw to do framing work. The saw was connected to an extension cord, which was routed to a temporary outlet device installed on the 3rd floor. The temporary outlet was energized and the outlet box did not have a cover. On or about 02/25/14.     NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM.  FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.

Employees were exposed falling approximately 10 feet through holes to the 2nd floor. On or about 02/25/14.     NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM.  FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
