29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees engaged in manufacturing activities in the following locations using the equipment listed below were exposed to struck-by and caught-between hazards in that machine guarding was not provided: a) Mori Seiki SL25 CNC lathe, in the machine shop, the interlock at the door had been removed. b) Mori Seiki NL CNC lathe, in the machine shop, the interlock at the door had been removed. c) Cincinnati Milacron CNC mill, in the machine shop. the interlock at the door had been over-ridden. d) Central Machining drill press, in the overhaul department, was not equipped with a guard. e) Delta drill press, maintenance shop, was not equipped with a guard.

29 CFR 1910.213(h)(4): Radial saw(s) were not installed in a manner so as to cause the cutting head to return gently to the starting position when released by the operator: Employees engaged in maintenance activities using the Ryobi radial arm saw in the maintenance shop were exposed to struck-by hazards in that the radial arm saw did not return to its starting position upon being released.
