29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about 02-13-14, company did not provide required employee safety training in the operation of a Vermeer E900 Plus evacuator for employees who did not associate electrical hazards when operating the equipment to expose electrical facilities without wearing required personal protection equipment, reading the manual prior to operation, and without safety decals on the equipment.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) On or about 02-13-14, two employees that were potholing to expose a three-phase, single phase, and communication line were operating the suction tube and high-pressure wand on a Vermeer E900 Plus evacuator without wearing insulated gloves and boots, which resulted in one of the employees receiving an electric shock that resulted in an injury.
