29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified:  a)  for employees at 3145 John Young Parkway, Orlando, (FL), involved in auto spray painting activities and handling paints, thinners and other solvents, as observed on or about 2/26/2014.  Employees were provided with latex gloves and showed signs of permeation after just 52 minutes of use; glove permeation aliphatic isocyanates was verified using PERMATEC-Pads.  Latex gloves were not recognized by the manufacturer's safety data sheet as providing satisfactory protection.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use.  a) for the employees required to use a 3M half-face respirator (07192) with organic vapor and P95 cartridges while working at the painting areas located at 3145 John Young Parkway, Orlando (FL) , as observed on or about 02/26/2014.  The respirators were required to be used when handling hazardous chemicals such as but not limited to hardeners, thinners, primers and paints during auto body painting operations.  As indicated by paragraphs (i - (ix) above, elements required by the program include but are not limited to: medical evaluations, fit-testing and training.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a) for the employees voluntarily using half-mask respirators while working at the body shop areas located at 3145 John Young Parkway, Orlando (FL), as observed on or about 03/27/2014.  The employees were provided SAS Safety (8661-92) Bandit Half Mask Respirators with organic vapor and N95 cartridges, for use at their discretion.
