29 CFR 1926.403(b)(1): Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees.  Safety of equipment shall be determined on the basis of the following considerations:  a) On or about February 28, 2014 at the jobsite located at 125 E. 133rd Street, Riverdale, IL. - The employer did not ensure that the temporary electrical wiring was free from hazards that were likely to cause death or serious physical harm. The hazards included, but were not limited to improper installation of electrical parts, exposed panels, wiring, and damaged electric cords, thereby exposing the employees to electrical hazards or shocks.

Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.   a) On or about at February 28, 2014 the jobsite located 125 E. 133rd Street, Riverdale, IL. - employees performing carpentry activities in rehab  buildings were using electric extension cords that were not protected from damage or pinch points due to the cords to power portable tools running across walkways and doorways, thereby exposing the employees to electrical hazards.

The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors:   a) On or about February 28, 2014 at the jobsite located 125 E. 133rd Street, Riverdale, IL. - employees performing carpentry activities in rehabs were using electrical equipment, including but not limited to hand tools and radios that were not protected by a ground-fault circuit interrupted or an assured equipment grounding conductor program, thereby exposing the employees  to electrical hazards.

The stairway landing was not providedd with guardrails, thereby exposing employees to fall hazards of approximately five (5) feet.
