29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. a) 2nd floor, north side bathroom: Employees used an electric powered reciprocating saw to do plumbing work. The saw was connected to an electric extension plug, which was connected to an extension cord and routed to a nearby electric outlet. The extension plug was missing a ground pin. On or about 02/25/14. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.

The stairway rise to about 10 feet height and was not equipped with a stairrail system along its unprotected side. On or about 02/25/14.
