The saw was connected to an electric extension plug, which was connected to an extension cord and routed to a nearby electric outlet. The extension plug was missing a ground pin. On or about 02/25/14.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM.  FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
