OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from a struck-by hazard from a metal coil. On or about February 28, 2014,in the Production Area, a machine operator was exposed to a struck-by hazard while operating a one ton Albel-Howe overhead underhung electric hoist with a metal coil attached and an inoperative safety latch on the hook.

29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. On or about February 28, 2014, a machine operator was exposed to a struck-by hazard while temporarily storing an unsecured 700 lb. metal coil against a wall in order to take off the existing metal coil on the OvalFormer Machine.

On or about February 28, 2014, in the Production Area, a machine operator was exposed to a struck-by hazard while wrapping the six foot synthetic sling around the 700 lb. metal coil to be lifted and placed on the coil reel of the OvalFormer Machine.
