29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about February 27, 2014, at 4094 Harper Franklin Ave, Augusta, Georgia:  a) Two employees performing framing work were exposed to struck-by hazards from falling tools, and materials from employees working above them from the second floor of the house when the employees worked on the ground without wearing head protection.   b) The same two employees performing framing work were exposed to struck-by hazards from falling tools, and materials from the employees working from the top plate while they worked beneath the employees on the second floor of the house and did not wear head protection.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about February 27, 2014, at 4094 Harper Franklin Ave., Augusta, Georgia:  Two employees framing a house were exposed to struck-by and impalement hazards from wood chips and nails when they used nail guns without wearing eye protection.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about February 27, 2014, at 4094 Harper Franklin Ave., Augusta, Georgia:   a) Four employees performing framing work were exposed to a fall hazard that ranged from approximately 10.6-feet to 11-feet when they worked at the unprotected edges of the house without using fall protection.    b) Two of the same employees performing framing work were exposed to a fall hazard of approximately 8-feet to the lower level and approximately 20-feet to ground level when they worked from the top plate of the house without using fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about February 27, 2014, at 4094 Harper Franklin Ave., Augusta, Georgia:  Four employees performing framing work were exposed to a fall hazard of approximately 11-feet when they used an approximately 10-foot long aluminum ladder that could not extend three feet above the second floor landing.
