Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being caught in the chipper.                    a) At the job site - on or about February 28, 2014 and at times prior to, employees were exposed to caught-in hazards associated with not being able to instantly stop the Woodchuck chipper's hydraulic functions from outside the feed horn by grabbing the panic bar. 29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:     a)  Jobsite, vehicle mounted work platform, on or about February 28, 2014, the employer did not ensure employees working from a vehicle mounted GMC HI-Ranger XT SS non-over center articulating aerial device S/N 2060430091 use fall protection, thus exposing the employee to struck-by and fall hazards up to about 55 feet.
