29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: On or about February 27, 2014, Rig #6 where the stairway came together with the rig-working platform, both sides of the railing had open gaps 12-inches wide, employees were exposed to a fall hazard greater than 6-feet.

29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs: On or about February 27, 2014, Rig #6, the second rung on the stairway leading up to the rigs working platform was damaged. The rung was caved in the middle and broken on both ends. Employees were exposed to slip and trip hazards.
