29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: On or about February 27, 2014, Capstar Rig #106, the employer did not ensure that suitable facilities for quick drenching were available to employees who were exposed to injurious corrosive materials such as but not limited to Methanol and Xylene. The worksite quick-drench emergency shower was not operational at the time of inspection; employees were exposed to eye, skin and inhalation chemical hazards.

29 CFR 1910.178(m)(5)(i): When the powered industrial truck was left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, and brakes were not set: On or about February 27, 2014, Capstar Rig #106 at the mixing area, while the inspection was taking place, the Forklift (Genie Model No. GTH 844, Serial No. GTH 0812-15415) used by the Boom Hand was left unattended, running with the load suspended 3 to 5 feet above the ground. Employees were exposed to struck-by and caught-between hazards.

Employees were exposed to shock hazards.
