OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to lacerations from jets of water during high pressure cleaning: In the following instances employees were not protected from lacerations and eye injuries caused by jets of water during hydroblasting operations: a) Pulp Mill - On or about February 25, 2014, the operator did not have direct control of the dump system. b) Pulp Mill - On or about February 25, 2014, a positive method (such as rigid pipe or safety guard) was not used to prevent the nozzle from reversing direction. c) DELETED d) Pulp Mill - On or about February 25, 2014, the hose was not marked (no closer than 24 inches from the nozzle) to warn the operator of the nozzle location. e) Pulp Mill - On or about February 25, 2014, hydroblasting was not stopped when team members (other than the operator) were located in the area. f) Pulp Mill - On or about February 25, 2014, the area around the hydroblasting operation was not barricaded or taped off.
