OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: On or about February 27, 2014, and at times prior thereto, employees were working on a rig site, Chocktaw Services Rig #11. The employer did not ensure that each anchor had a visible tag securely attached to it which indicated the capacity of the anchor and the date of the capacity verification. Two of four anchors did not have verification tags. Employees were exposed to potential struck-by, crushing and structural collapse hazards.
