29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:    On or about February 25, 2014, Rig #22, at the Discharge pit, a floor hole measuring approximately 10-inches by 8-inches was left unguarded. Employees were exposed to trip hazards.

29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    On or about February 25, 2014, Rig #22, the stairway leading up to the discharge pit had unprotected gaps on each side. The unprotected gap on the left side was approximately 2-feet wide and the unprotected gap on the right side was 5-feet 5-inches wide. Employees were exposed to a fall hazard of greater than 4-feet.

29 CFR 1910.27(b)(1)(ii): The distance between rungs exceeded 12 inches and were not uniform throughout the ladder:      On or about February 25, 2014, on the rig floor of Rig # 22, the ladder leading up to the monkey board was cut 41-inches above the floor and in other areas to accommodate the water valves. This caused the distance between rungs to exceed 12 inches in several areas.  Employees were exposed to the fall hazards greater than 4-feet.

29 CFR 1910.27(b)(1)(iv): Rungs, cleats, and/or steps of fixed ladders were not free of splinters, sharp edges, burrs, or projections that posed a hazard:    On or about February 25, 2014, on the rig floor of Rig # 22, the ladder leading up to the monkey board was obstructed by the water valves. Employees were forced to step on the valves in order to access the ladder. This exposed employees to the fall hazards greater than 4-feet.

Employees were exposed to electrical shock.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors:    On or about February 25, 2014, Rig #22, 110/120V and 220V nominal electrical cables were run through holes in walls. Employees were exposed to electrical shock hazards in the following instances:          1)    Six 120V electrical cables were run through the metal walls of the Generator House.      2)    Two 110V & two 220V electrical cables were through walls of the Top Doghouse.      3)    Two 110V cables were run through the metal roof brackets of the Bottom Doghouse.
