29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: On or about February 26,2014, and at times prior thereto, employees were working at a rig site, H & P Rig #347. The employer failed to maintain the pre-mix pit area in a clean and orderly condition. The employees were exposed to slip and trip hazards.
