29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation: On or about February 25, 2014, Rig #22 near the Discharge pit, a 25-foot yellow electrical extension cord used to supply power to the gas agitator was damaged to the point of adversely affecting safe operation. The extension cord had a 5-inch section where both outer insulation and insulation protecting the live conductors was scorched through. Employees were exposed to electrical shock hazards.

29 CFR 1910.305(g)(1)(i): Flexible cords and cables were not approved or conditions of use and location: On or about February 25, 2014, Rig #22 near the Discharge pit, two 25-foot electrical extension cords were plugged together to supply power to the gas agitator was damaged to the point of adversely affecting safe operation. One extension cord had a 5-inch section where both outer insulation and insulation protecting the live conductors was scorched through. The employer did not ensure that employee(s) followed manufacturer's instructions. Employees were exposed to electrical shock hazards.
