29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. On or about February 25, 2014 employees were exposed to tripping hazards while working on an elevated surface. Employees utilize the steps leading from the ground surface up to the hopper landing on a routine basis.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (iv) of 29 CR 1910.303(b)(1): On or about February 25, 2014 employees were exposed to electrical hazards while performing work duties on or near the accumulator. Electrical cable in excess of 480 volts originating from the power generation annex (light house) to the accumulator have burnt surfaces.
