29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:   On or about March 4, 2014; on the west side of the jobsite, the employer allowed employees to utilize an extension cord that was missing a ground pin.  The extension cord was used to deliver power from a temporary power distribution panel to a kerosene/diesel forced air heater.  The missing ground pin prevented the extension cord from having a continuous path to ground.
