29 CFR 1918.98(b)(2): Each immediate supervisor (assigned after July 16, 1999) of a cargo handling operation of more than five persons, did not satisfactorily complete a course in accident prevention within 90 days of that assignment: a) Hanjin Matsue deck and holds, Dock and loading/storage area - On or about February 28, 2014, the employer did not ensure a supervisor who had completed a satisfactory accident prevention course was present during the loading of scrap metal by a crew of six, when an employee fell into the hold and was seriously injured.
