29 CFR 1910.146(k)(1)(ii): When designating rescue and emergency services pursuant to paragraph (d)(9), the employer did not evaluate the prospective rescuer's ability, in terms of proficiency with rescue-related tasks and equipment, to function appropriately while rescuing entrants from the particular space or types of spaces identified:      A.)  Blair Strip Steel Company - On or about 3/19/14, the employer had not evaluated the rescuer's ability to perform a confined space rescue from all of the confined spaces in the facility.  Personnel enter confined spaces that included, but were not limited to, the mill stand pit, the water meter pit, and the set pit.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:      A.)  Blair Strip Steel Company - On or about 3/19/14, the company had not conducted an annual inspection of the lockout/tagout procedures with all authorized employees.  Personnel conduct servicing and maintenance on machinery/equipment including but not limited to, furnaces and the oscillating winder.

29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:      A.)  Blair Strip Steel Company - On or about 3/19/14, the employees utilized a man basket attached on a forklift without approval from the forklift manufacturer.   The forklifts which the man basket was used on included but was not limited to, various electric Yale forklifts.  The man basket was used on the forklifts for activities that included, but were not limited to, changing light bulbs.

Blair Strip Steel Company - On or about 3/19/14, the company was using the space in front of a 120/240 volt panel box for storage of oil, rags, and coil bands.  The electrical panel supplied power to the overhead lights.
