29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     (a) On or about February 27, 2014, at worksite located at 9402 NE 93 Rd Kansas City Missouri.- An employee was exposed to serious struck by injuries, while working under roofing activity from approximately 15 feet above and not wearing a protective helmet to protect from potential falling wood, working tools and pneumatic equipment.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.       (a)  On or about February 27, 2014, at worksite 9402 NE 93rd Kansas City Missouri. - An employee was exposed to a fall hazard by using a ladder which extended approximately 2 feet above the landing surface of the roof to which he gained access.
