29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  On or about 2/27/2014, at the above addressed jobsite, an employee was engaged in tree cutting work from a bucket truck approximately 18 feet above ground level without a means of fall protection, exposing him to a fall hazard.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:   On or about 2/27/2014, at the above addressed jobsite, an employee was engaged in palm trimming work from a bucket truck with no hardhat, exposing him to a struck-by hazard.
