Within the production area, on or about 03/04/14: Employee operating the Bridge Machine Company, Titan BT001, meatball former without the hopper, thereby being exposed to a rotating auger. b. Within the production area, on or about 03/04/14: Employee operating the Orics industries, Inc., R-20, packaging machine did not have the point of operation guarded in that an interlock switch attached to the side door was bipassed, thereby being exposed to rotating parts. Abatement certification must be submitted for these items.
