29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.: a) For the jobsite at 3300 Bonita Beach Rd., Bonita Springs, FL 33716, as observed on or about 02/25/2014, employees were exposed to fall hazards of 21.25 ft. while walking/working off of a scaffold platform that was missing guardrails.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used: a) For the jobsite at 3300 Bonita Beach Rd., Bonita Springs, FL 33716, as observed on or about 02/25/2014, a ladder for safe access and exiting of scaffolding was not present. Employees working on scaffolding were observed to be climbing the cross braces of the scaffold.
