29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Cutting Room, Planer: A worker did not utilize lockout tagout procedures while attempting to free a stuck block of fiberglas at the planer. A worker's fingers were pulled into a conveyor belt and sprocket at the planer and the worker was injured.

Planer: The back side guard of the planer machine was not secured to the machine and employees were exposed to an amputation hazard. The machine could be started with the cover opened and the back side guard could be opened while the machine was running.
