29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:      a) Employees were exposed to eye injury while operating a nail gun without the eye protection during roof sheathing work from a pitched roof surface.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:     a) Employees were exposed to the hazard of falling approximately 13 feet while working from scaffolding without the benefit of fall protection. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):          a) Employees were exposed to the hazard of falling while performing carpentry work from a 20/12 pitched roof without the use of fall protection.          b) Employees were exposed to the hazard of falling while performing carpentry work where there were window and wall openings at the second story level of the building.      c) Employees were exposed to the hazard of falling while performing carpentry work where there was an unprotected floor hole on the second story of the building.          In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
