29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Site - On or about 02/26/14 employees were installing tar paper and elevating/distributing  shingles on top of the roof without fall protection equipment, and were exposed to falls from a maximum elevation of 17.0 ft. to ground level.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  a) Site - On 02/26/14 the employer instructed the workers  to transport roofing materials with an elevator (mounted on on an unsecured aluminum ladder), situated in proximity of  an unprotected power line. Employees were exposed to accidental contact with electrical energy.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  Site - On or about 02/26/14 employees were installing tar paper and elevating/distributing shingles on top of the roof without fall protection equipment, and were exposed to falls from a maximum elevation of 17.0 ft. to ground level. The employer did not certify that the fall protection training was completed and maintained up to date.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
