29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. (a) Scaffold - On or about February 20, 2014 employees were exposed to head injuries while working beneath scaffold. Employees were handling bricks and mortar above the ground level employees. One employee was observed throwing bricks up to 14 feet to employees on scaffold. Employees were not wearing hardhats.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. (a) House, Left-Side - On or about February 20, 2014 three employees were exposed to a fall hazard while working on a scaffold that had no ladder installed. Employees climbed scaffold frame 7 feet 5 inches above the ground to access the scaffold.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components was not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. (a) House, Left-Side - On or about February 20, 2014 three employees were exposed to a fall hazard due to the fact the scaffold was not inspected by a competent person prior to each work shift. Scaffold did not have a ladder installed prior to use.
