29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds  was not fully planked or decked between the front uprights and the guardrail supports as follows:   (a)  Scaffold - On or about February 25, 2014 an employee was exposed to a fall hazard from a scaffold not being fully decked or planked. Only one deck was installed on the scaffold. Employee would fall 13.8 feet to broken concrete.

(a)    Scaffold - On or about February 25, 2014 an employee was exposed to a fall hazard while standing on a scaffold 13.8 feet above concrete with no base-plates installed.       (b)   Scaffold - On or about February 25, 2014 three employees were exposed to a fall hazard while standing on a scaffold 9 feet above concrete with no base-plates installed.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.    (a)  Scaffold - On or about February 25, 2014 an employee was exposed to a fall hazard while standing on a scaffold 13.8 feet above concrete. Employees were not protected from falling to a lower level by a guardrail system or personal fall arrest system or safety net system.

29 CFR 1926.452(w)(1): Scaffolds was not braced by cross, horizontal, or diagonal braces, or combination thereof, to prevent racking or collapse of the scaffold and to secure vertical members together laterally so as to automatically square and align the vertical members. Scaffolds shall be plumb, level, and squared. All brace connections were not secured.   (a)  Scaffold - On or about February 25, 2014 employees were exposed to a fall hazard of 13.8 feet while working on a scaffold that was missing cross-braces on the front and back-side.
