29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold shall be protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).        a)         Diversified Builders Inc. at 1550 Market Street, Denver, CO 80204: On and before 2/3/14, the exposing employer did not ensure that each employee performing work from a supported scaffold was protected from falling from all sides and ends of the scaffold had guardrails. A guardrail or other means of fall protection was absent from a section of the scaffold on the south side of the building. This condition exposed employees to a fall hazard of over 4 stories high. 29 CFR 1926.451(g)(3)(i): When vertical lifelines are used, they shall be fastened to a fixed safe point of anchorage, shall be independent of the scaffold, and shall be protected from sharp edges and abrasion. Safe points of anchorage include structural members of buildings, but do not include standpipes, vents, other piping systems, electrical conduit, outrigger beams, or counterweights.    a)         Diversified Builders Inc. at 1550 Market Street, Denver, CO 80204: On and before 2/3/14, the exposing employer did not ensure that when vertical lifelines were used, they were independent of the scaffold. Employees were working on the exterior of a multistory building. Employees were working on the scaffold tied off to the member of the scaffold. The employer did not ensure the lifeline was independent of the scaffold. This condition exposed employees to a fall hazard of over 4 stories high. 29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  a)  Diversified Builders Inc. at 1550 Market Street, Denver, CO 80204: On and before 2/3/14, the exposing employer did not ensure that anchorages associated with fall protective systems were adequate and would support an employee in the event of a fall. The personal fall arrest system was anchored to the component of the scaffolding system. In the event of a failure, this condition exposed employees to a fall hazard of over 4 stories high.
