29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      a) On February 20, 2014 and at times prior thereto; roof area; employees were observed working on a multi-slope residential roof without attaching the lanyard of the personal fall arrest system to the D ring or not wearing a fall protection device to prevent them from falling 35-40 feet to ground level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder�s length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed     a) On February 20, 2014 and at times prior thereto; roof area; the extension ladder used to access the work area (roof) was set 6 inches above the working platform exposing them to a fall hazard of 25 feet to ground level while reaching for the steps or platform.

29 CFR 1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a) On February 20, 2014 and at times prior thereto; roof area; employee was carrying material up a ladder such as but not limited shingles that could cause the employee to lose balance and fall 25 feet to ground level.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
