29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. On or about February 27, 2014, on the north side of the construction site, employees installing exterior insulated finish systems, used a work platform that was not approved by the forklift manufacture, exposing employees to a fall hazard of approximately 13 feet.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied. On or about February 27, 2014, on the north side of the construction site, employees installing exterior insulated finish systems (EIFS), worked from a work platform that was not securely attached to the forks of a 10042 JLG forklift that was moved while occupied, exposing employees to a fall hazard of approximately 13 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section. On or about February 27, 2014, on the north side of the construction site, employees working from a work platform elevated by a 10042 JLG sky track installing exterior insulated finish systems (EIFS) on a commercial building were exposed to a fall hazard of approximately 13 feet.
